NOTICE:  Legacy Martial Arts strongly encourages that all applicants obtain a physical examination from their doctor prior to attendance in any exercise or martial arts class.

ANY ACTIVITY WHICH INVOLVES THE MARTIAL ARTS MAY, AS IN ANY SPORT, CAUSE ACCIDENTAL INJURY.

I, {name}, hereby apply to Legacy Martial Arts for martial arts instruction for myself and/or my dependent.  If accepted, I agree to pay all fees as required by Legacy Martial Arts.  In recognition of possible injury connected with physical activity, I hold Master Jena Bushéy, Mr. Jack Smail, Legacy American Martial Arts Inc., its staff and the owner of the school facilities and equipment thoroughly and entirely harmless from any and all claims of any nature.  In case of emergency, I hereby authorize Master Jena Bushéy, Mr. Jack Smail, or their representative to secure emergency medical care.

On behalf of myself or the above named minor, I waive, release and discharge any claims arising out of or relating to the use of photographs and/or audio or video recordings made at Legacy Martial Arts or at any event participated in by Legacy Martial Arts in which I or the above named minor may feature. Legacy Martial Arts is not responsible for any photographs and/or audio or video recordings taken by spectators at classes or other events.

  • Wear active wear for the first lesson. 
  • We are located at 1911 Robert Street South in Southview Square between the Erbert & Gerbert's and O'Reilly's. 
  • If the student is under 18, make sure a parent or guardian is present for the entire lesson. They’re not going to want to miss the 1st lesson!
  • We'll have a staff member waiting for you, so if for any reason you can't make it, please let us know and we can reschedule.
